Your health summary

You should have received a copy of your health assessment before you left care. If you do not have your health summary, speak to your personal adviser. 

Your health assessment includes useful information such as:

• How to access your health history from your GP
• Details of your immunisations

It is your right to have this record of what you were treated for in the past.

Make sure you read your health summary and keep it safe.
