How we assess eligibility for Short Breaks

We assess eligibility for Short Breaks using the information and supporting evidence you provide with your Short Breaks application.

Supporting evidence must include a range of recent reports from professionals, such as health reports and education reports. These should ideally be from within the last 12 months. The reports must show the impact your child's disability has on their everyday life and family life.

A Children with Disabilities (CWD) social worker or Short Breaks team member takes the information and scores it against a Resource Allocation System (RAS) questionnaire.

We evaluate every child using the same questionnaire and criteria. This ensures that every family receives fair and equal resources.

The higher the score on a RAS questionnaire, the more likely your child needs more specialist Short Breaks provision.  

Applying for Short Breaks for more than one child

You must complete an application for each child. We will complete a separate RAS questionnaire for each child.
