Proof of Identity and Residence
We will need proof of your identify and residence before we can release any information. You will need to upload an electronic copy of one of the following documents when you reach the relevant section within this online form:

- Current signed passport
- Current driving licence

- Adoption certificate
Proof of residence at your current address:
- Gas, electric, or water bill from the preceding 6 months
- BT/cable fixed line telephone bill from the preceding 6 months
- Council tax statement from the current year
- Bank or building society statement from the preceding 6 months
Acting on behalf of someone else
If you are requesting information on behalf of someone else we will also need proof that you have authority to act on their behalf, for example, their written consent or lasting power of attorney.
If acting on behalf of a child, you will need to provide evidence of parental responsibility, for example a copy of the birth certificate. Please note, that without the relevant authority, it is likely that your request may be refused.
