Claims for NHS health checks, sexual health services and stop smoking services
This is Norfolk County Council's portal to claim for activity provided under the NCC Primary Care Contract.
Contracted general practices can submit activity claims to Norfolk Public Health for:
- NHS health checks
- Sexual health services (LARCs)
- Stop smoking services

Invoices
After submitting a form you'll receive an email with the amount to claim and the purchase order number. You'll need to include the purchase order number in your invoice.
Email invoices to invoices@norfolk.gov.uk
Note: We'll send separate emails for chlamydia positive payments.
Submission dates
The form won't time out, but the following cut-off dates per quarter apply. If you need to submit data for a previous quarter, please add it to this current quarter's submission.
We've listed the cut-off dates below.
Quarter 1: 1 April to 30 June
- Health checks cut-off date: 20 July
- Sexual health cut-off date: 20 July
- Stop smoking cut-off date: 22 August
Quarter 2: 1 July to 30 September
- Health checks: 20 October
- Sexual health: 20 October
- Stop smoking: 22 November
Quarter 3: 1 October - 31 December
- Health checks: 20 January
- Sexual health: 20 January
- Stop smoking: 22 February
Quarter 4: 1 January - 31 March
- Health checks: 20 April
- Sexual health: 20 April
- Stop smoking: 22 May
