Our aim for anyone needing end of life care is making them as comfortable as possible in their final years.
You have the right to choose where you want to get end of life care and where you die.
Professionals will identify and assess end of life care needs. The care you receive might need adjusting as your needs change. Services can also change and develop to ensure carers get the support they need.
Some people at the end of their life might be eligible for NHS continuing healthcare. There is a fast-track assessment process to provide a quick response.

Thinking ahead with an advance statement
An advance statement is something you write about your future care needs. It sets out the treatment decisions you have made and the care you wish to receive as you reach the end of your life. It is a personal plan which you can share with your family and health and social care workers so they know your wishes.
