What is in the plan

Positive outcomes

When we confirm your child is eligible for Short Breaks, we will discuss and agree their plan with you. We will base the plan on their identified needs and outcomes. An outcome is something your child wants to achieve by taking part in the activity. Every child is different, so your child's outcomes will be unique to them. 

The plan will include types of services, activities or items to meet these needs and outcomes.

This may be one or more of the following:

  • Commissioned services. These are specialist services from approved providers that we can arrange for you.
  • Personal assistant (PA). This is a person employed by you to support your child in the community or in the family home. Our Direct Payment Support Service (DPSS) will support you to employ them.
  • Pre-paid debit card. You can use this to buy activities and/or items that we agree in your child's plan.

Changing your Short Breaks plan

There is high demand for the Short Breaks service. We cannot make changes to your child's Short Breaks plan unless there is a significant change in your child's needs.

We are unable to make any changes to your child's plan within the last three months of the plan ending.

Please be aware that at busy times there may be a delay to changing the plan even if it is possible.
